The Lakeshore Nurse Practitioner-Led Clinic (LCNPLC) provides accessible, patient-centred, and equity-driven primary care. Under a co-leadership model, the Executive Director – NP collaborates closely with the Executive Director – Administration to ensure the clinic delivers high-quality services, supports interprofessional collaboration, and meets the Ontario Ministry of Health’s standards for Nurse Practitioner-Led Clinics. Reporting to the Board, the Executive Director – Nurse Practitioner is accountable for clinical service delivery, professional practice standards, interprofessional team leadership, and quality improvement. This is a dual role that combines maintaining a clinical caseload with leading the clinic’s strategic and operational initiatives. The position plays a key role in advancing health equity, strengthening community partnerships, and fostering a collaborative, inclusive workplace culture.

What You’ll Be Responsible For:
- Clinical Leadership: Oversee primary care delivery, maintain a clinical caseload, develop clinical policies, and champion evidence-based practice.
- Team Management: Supervise, mentor, and coach NPs, RNs and other clinical staff; lead recruitment, onboarding, and performance management.
- Strategic Alignment: Translate strategic priorities into operations in collaboration with the Executive Director – Administration.
- Quality Improvement & Governance: Lead annual Quality Improvement Plans, monitor KPIs, ensure compliance with regulations, and report to the Board and Ontario Health.
- Community Engagement: Build strong relationships with system partners, advocate for nurse practitioner-led care, and support health promotion initiatives.
The Ideal Candidate:
- Current registration and in good standing with the College of Nurses of Ontario (NP–Primary Health Care).
- Graduate degree (Master’s level) in nursing or a related health discipline.
- 5+ years of clinical NP experience in primary/community care settings.
- 3+ years of leadership experience.
- Experience in an interprofessional primary care team.
- Proven expertise in clinical governance, mentorship, and quality improvement.
- Strong knowledge of Ontario’s primary care system, social determinants of health, and equity-focused practices.
- Demonstrated ability to lead anti-racist, anti-oppressive initiatives and engage effectively with priority populations.
- Proficiency in EMRs (PSSuites) and clinical data tools preferred.
- In-depth understanding of the NPLC model.
- Commitment to the Alliance for Healthier Communities Health Equity Charter.
Complimentary Experience:
- Bilingual in French/English.
- Experience with community governance boards and Ministry reporting.
- Familiarity with Lakeshore’s community health and equity priorities.
Additional Requirements:
- Flexibility for evening and weekend work.
- Valid Ontario driver’s license and reliable transportation.
- Commitment to organizational policies, patient safety, and equity-driven care.
This position offers a comprehensive compensation and total rewards package including:
