The STAR Family Health Team is recruiting to all 3 of their locations in Stratford and Tavistock.
The Avon site (Stratford) includes 6 Family Physicians;
The O'Loane site (Stratford) includes 4 Family Physicians;
The Tavistock site includes 4 Family Physicians.
Collectively, the STAR Team includes:
- 14 Family Doctors
- 4 Nurse Practitioners
- 15 Nurses
- 5 Mental Health Clinicians
- 2 Registered Dieticians
- 2 Pharmacists
- 19 Administrators

- Opportunity available to join an existing clinic with established colleagues.
- Physicians establishing a practice are eligible for income stabilization.
- Physician owned and operated clinics.
- TELUS PSS EMR with Ocean/e-Referral platform and option for online booking.
- After hours clinic/call is approximately 1 in 14 weekdays.
Stratford is home to an excellent and collegial specialist base with access to tertiary care centres at LHSC and KW area for additional support.
New FHO physician will be expected to maintain a roster of ~1000 patients or more.
Accommodation is available in Stratford for locum physicians.
