The Opportunity:

Under the direction of the Therapeutic Lead, Clinic Solutions, the role of the Practitioner is responsible for the delivery of patient services offered by Sentrex Health Solutions. The Practitioners main responsibilities include, but not limited to managing day to day patient care for assigned clinics. This role involves direct interactions with patients, and collaboration with members of an interdisciplinary health team such as family physicians, therapists, pharmacists, insurance companies, and other health care professionals (HCP’s) to plan, implement coordinate and evaluate patient care in consultation with patients and their families, as well as support the day-to-day operations and expansion of the program through the development of professional relationships with key stakeholders and community providers within selected geographies.

Clinical Responsibilities:

  • Has professional and managerial responsibility for a caseload of patients in which they are responsible for decision making and clinical judgement to formulate appropriate treatment plans in consultation with affiliated physicians
  • Perform a patient advocacy role
  • Prescriber and administer medications
  • May co-ordinate or manage primary patient care

Operational Responsibilities:

  • In partnership with the senior leadership team, develop, monitor, and lead the implementation of the clinical services strategic and operational plans, and provide input and monitor applicable financial budgets.
  • Oversee the creation and implementation of program objectives, action plans, along with benchmarks and evaluation procedure that align with the strategic and operational plans of the business.

A Day in the Life (What you will do here):

  • Diagnosing, Assessing, Managing patients.
  • Process data entry activities of pertinent information received at different points of contact during the delivery of services
  • Provide medical support to patients and HCPs, including but not limited to education on specific diseases, therapies, and disease management.

What you need to ensure you are set up for success:

  • Minimum 2-4 years of clinical experience (preferred)
  • Licensed Nurse Practitioner or General Practitioner with experience in geriatrics, neurology or GAIN/GEM
  • Must have an active license to work in Ontario
