Work Type: Full Time

Position Summary

  • The Nurse Practitioner (NP) is an integral member of the renal health care team in the provision of comprehensive health care services.
  • Responsibilities of the role include coordinating the plan of care for a renal patient population through modality transitions with a focus on independent dialysis education.
  • Facilitate and/or provide independent dialysis mentorship to various groups, including staff in the Regional Renal Program, hospital, long-term care facilities and community care providers (regulated and non-regulated) and/or medical residents, in order to promote and encourage the use of independent dialysis modalities for appropriate patients.


Position Requirements

  • Successful completion of Masters prepared NP program with a minimum of 2 years current NP experience in Acute Care and renal experience OR successful completion of two year university level NP program (Extended Class) with a minimum of 5 years hemodialysis experience required.
  • BCLS and current registration RN (EC) with College of Nurses of Ontario in good standing.
  • CNA Certification of Nephrology (CNeph © preferred).
  • Effective communication and leadership capabilities.
