About Hospital at Home

The Hospital at Home program, based at St. Paul’s Hospital (SPH), is a provincial initiative that delivers acute-level care to eligible patients within a 15-minute driving radius of SPH. It serves patients with acute medical conditions who are clinically stable enough to be safely cared for at home.

Care includes:

  • Remote patient monitoring (including vital signs and a call bell system)
  • In-home visits and virtual assessments (via Zoom or phone)
  • In-hospital assessments and procedures when necessary
  • Access to point-of-care ultrasound during home visits
  • Transportation provided for home visits

Position Overview – Registered Nurse, Hospital at Home

Reporting to the Patient Care Manager, the Registered Nurse will:

  • Deliver personalized care in patients' homes or hospital settings
  • Assess patient needs and develop individualized care plans
  • Implement interventions to support recovery and well-being
  • Foster a safe and respectful environment for patients and families
  • Collaborate with a supportive, interdisciplinary team
  • Share clinical expertise and contribute to continuous learning
  • Maintain the highest standards of nursing practice

Common Patient Diagnoses:

  • Congestive heart failure
  • Pneumonia
  • COPD
  • Dehydration
  • Sepsis
  • Cellulitis

What You Bring

Education, Training and Experience

Education: Registered Nurse with BC College of Nurses and Midwives registration.

Experience: Minimum of one year of acute care nursing experience.

Other: Valid BC driver’s license for local travel.

Skills and Abilities

  • Ability to communicate effectively both verbally and in writing.
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
  • Ability to work as a member in an interdisciplinary team.

  • Physical ability to carry out the duties of the position.
  • Ability to prioritize and organize work.
