About Hospital at Home
The Hospital at Home program, based at St. Paul’s Hospital (SPH), is a provincial initiative that delivers acute-level care to eligible patients within a 15-minute driving radius of SPH. It serves patients with acute medical conditions who are clinically stable enough to be safely cared for at home.
Care includes:
- Remote patient monitoring (including vital signs and a call bell system)
- In-home visits and virtual assessments (via Zoom or phone)
- In-hospital assessments and procedures when necessary
- Access to point-of-care ultrasound during home visits
- Transportation provided for home visits
Position Overview – Registered Nurse, Hospital at Home
Reporting to the Patient Care Manager, the Registered Nurse will:
- Deliver personalized care in patients' homes or hospital settings
- Assess patient needs and develop individualized care plans
- Implement interventions to support recovery and well-being
- Foster a safe and respectful environment for patients and families
- Collaborate with a supportive, interdisciplinary team
- Share clinical expertise and contribute to continuous learning
- Maintain the highest standards of nursing practice
Common Patient Diagnoses:
- Congestive heart failure
- Pneumonia
- COPD
- Dehydration
- Sepsis
- Cellulitis
What You Bring
Education, Training and Experience
Education: Registered Nurse with BC College of Nurses and Midwives registration.
Experience: Minimum of one year of acute care nursing experience.
Other: Valid BC driver’s license for local travel.
Skills and Abilities
- Ability to communicate effectively both verbally and in writing.
- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
- Ability to work as a member in an interdisciplinary team.
- Ability to apply decision-making and analytical skills.
