Summary

The Hospital at Home program, based at St. Paul’s Hospital (SPH), is a provincial initiative that delivers acute-level care to eligible patients within a 15-minute driving radius of SPH. It serves patients with acute medical conditions who are clinically stable enough to be safely cared for at home.

Within the context of a client- and family-centered model of care, and guided by Providence Health Care’s Mission, Vision, Values, and strategic directions, the Registered Nurse, Hospital at Home:

  • Promotes a safe, respectful, and inclusive environment for patients, families, visitors, and staff
  • Assesses, plans, implements, and evaluates nursing care for patients with acute health needs in the home and/or hospital setting
  • Develops and revises individualized care plans, incorporating patient and family teaching and referrals to community resources as needed
  • Provides direct nursing interventions and documents care according to professional standards and PHC protocols
  • Collaborates with and supports colleagues by sharing clinical knowledge and demonstrating best practices
  • Works within an interdisciplinary clinical framework in alignment with BCCNM Standards of Practice and PHC values

What You Bring

Education

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
  • Minimum of one year of acute care nursing experience
  • Valid BC driver’s license and access to a personal vehicle for local travel

Skills and Abilities

  • Ability to communicate effectively both verbally and in writing
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
  • Ability to work as a member in an interdisciplinary team
  • Ability to apply decision-making and analytical skills
  • Ability to deal with others effectively
  • Ability to role model conflict resolution strategies
  • Physical ability to carry out the duties of the position
  • Ability to prioritize and organize work
  • Ability to operate related equipment
  • Ability to teach and identify patient/client learning needs
  • Ability to lead patient/family group sessions
