Description

Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of-the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduce innovative and inclusive programs and services and new models of care delivery. PCH is committed to building partnerships and connections that put people first, as well as a learning and leading healthcare community that provides best in class service across the aging continuum.

Key Responsibilities: As a Registered Nurse (RN) you will support PCH residents by assuming the following responsibilities:

  • Community Driven:
    • Organizes and co-ordinates Personal Care with hospital, home and community to meet individual, family and group needs
    • Establishes and maintains good human relations with professional and non-professional staff, residents, families, volunteers, and the community
  • Caring Service:
    • Assumes responsibility for medication administration, narcotic control, removal of discontinued, unused, or expired medication, application of treatments and dressings, transcription of Physician orders, reordering of medications, and other skills
    • Guides the health care team to provide effective care for all individuals on the Resident Home Area
  • Professionalism:
    • Provides input to the development and revision of Personal Care policies and procedures and communicates changes to staff
    • Participates in the Home programs (e.g.) Occupational Health and Safety Program, Quality Improvement, Fire Safety, WHMIS
  • Innovative Approaches:
    • Promotes a co-operative working environment
    • Facilitates team building and promotes change and innovative ideas

Qualifications:

  • Post-graduate studies in Geriatrics and Nursing Administration are an asset
  • Current certification of competence of the College of Nurses of Ontario
  • Current Medication Certificate which meets the College of Nurses Standards of Practice
  • One year experience in a long-term care facility with knowledge and interest in gerontology
  • Members R.N.A.O./Gerontological Nursing Association preferred
