Registered Nurse - C6 Integrated Community Program - Regular Full-time 2025-13401 (2025-13401)

Summary Of Duties

The RN supports the philosophy of person-focused care and principles and the mission and values of Sunnybrook Health Sciences Centre.

The RN works closely with the members of the interprofessional team to provide care including discharge planning.

Duties And Responsibilities Include

  • Patient focused care to medical, acute geriatric, short stay, surgical oncology, gynaecological oncology, and plastics
  • The main functions of the RN include but are not limited to:
  • Assessment, intervention, education, and patient advocacy
  • Provides patient-centered care within an interdisciplinary care team to meet the physical, psychological, and spiritual needs of patients in supporting their return to baseline functioning and discharge to the community
  • Demonstrate advanced communication, problem-solving, conflict resolution, and interpersonal/team building skills
  • Plans for, delivers, and evaluates nursing interventions, including medication administrations as well as treatment plans which fall within the RN’s scope of practice
  • Fosters ongoing learning for staff and students with a commitment to preceptorship
  • Integration of research findings into practice, participate in unit projects or organizational initiatives such as i-lead and best practice champion

Qualifications/Skills

  • Current registration with the Ontario College of Nurses
  • Registration in good standing
  • Bachelor of Science in Nursing preferred
  • Current B.C.L.S. required
  • Successful completion of pre-interview quiz and received passing mark on quiz
  • Recent experience in acute care medicine, minimum 2 years preferred
  • Surgical Oncology experience preferred
  • Nursing Oncology Certificate preferred
  • Computer literacy is an asset
  • Demonstrated ability to take professional accountability for leading, organizing, providing, and delegating care activities for the units, patients, and families
  • Demonstrated commitment to person-centered care concept and principles
  • Excellent interpersonal and communication skills
  • Excellent assessment, critical thinking, problem-solving, and clinical decision-making skills
  • Established good time management skills, with the ability to set priorities while working in a fast-paced environment
  • Demonstrated ability to work effectively as a team member in an interprofessional team environment
  • Demonstrated ability to implement and champion change
  • Demonstrated commitment to continuous learning and evidence-based practice
