CareRx is looking for a Clinical Consultant Pharmacist to join our Vancouver Island team. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long-term care, supportive living, and other congregate living communities. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.

Role Accountabilities
- Act as a liaison between long-term care homes and retirement communities and CareRx Pharmacy to ensure optimal Resident care, Pharmacy Services, and Clinical support.
- Travel to LTC homes and retirement communities.
- Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships.
- Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx Pharmacy.
- Complete all required documentation and billing processes as required by MOHLTC, College of Pharmacists of British Columbia, and other federal and provincial standards and regulations.
What You Will Bring To The Team
- Insured and currently licensed in good standing with the College of Pharmacists of British Columbia.
- Proven ability to foster strong partnerships with other members of the healthcare team.
- Effective organization, planning, and time management skills.
- Knowledge of Kroll Windows application and Microsoft Office programs.
- Complete understanding of provincial regulations governing LTC homes and Retirement Communities.
Compensation Range: $109,200-$119,600 annually (based on a 37.5-hour workweek). Actual pay rate will be based on the individual candidate’s credentials and experience.
Location: Location will primarily be LTC homes and retirement communities in the Vancouver Island, preferably in the eastern coasts of the Island.
Work Shifts: This is a full-time position with flexibility and opportunity to work-from-home and on-site from the care homes. Work schedule focused within the standard Monday to Friday 9am-5pm.
