What will you do?

  • The Physiotherapist plans, organizes, and directs diverse programs of treatment, counseling, screening, and education for ONTARIO HEALTH ATHOME Champlain Patients in a variety of settings (in homes and retirement homes)

Primary Responsibilities

  • Provides physiotherapy services to ONTARIO HEALTH ATHOME Champlain patients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up, and management of the patient’s condition.
  • Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.
  • Ensures that clinical records are maintained in accordance with ONTARIO HEALTH ATHOME Champlain Policies and Procedures and the College of Physiotherapists of Ontario.
  • Schedules patient visits according to Physiotherapy priorities.

Hours of work: 8:30am-4:30pm – Monday to Friday

Starting Salary: $44.480 to $51.340

Reporting to: Manager, Patient Services

Office Location: Labelle, Smith Falls, Pembroke, Renfrew

Caseload Geography: Pembroke

What must you have?

  • Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
  • Related professional experience which would normally be attained over a period of two years in a Physiotherapy role
  • Related experience in a similar community-based setting is an asset

What would give you the edge?

  • Knowledge of health care system and the role of the Ontario Health atHome Champlain within the system
  • Effective planning and organizing skills to manage multiple patients, provide information reports, and take corrective action as required
  • Ability to deliver information effectively in a variety of settings including one-on-one, team meetings, and to assist in delivering presentations/information regarding the ONTARIO HEALTH ATHOME Champlain Physiotherapy program

Hours of Work Occasional shifts: Monday to Friday – 8:30am to 4:30pm
