Summary:

Bayshore Integrated Care Solutions

The Physiotherapist, under the direction of the Manager of Rehabilitation or Clinical Manager- Branch level, is responsible for completion of professional activities that include: assessment, treatment, education, development of home exercise programs, supervision of Rehabilitation Assistants (RAs), and management of clients requiring Physiotherapy intervention, in accordance with professional standards.

16 week home rehab program with a focus on maintaining client’s safety and well-being in their homes utilizing a client-centered approach.

Responsibilities:

  • Assessment of client’s mobility in home settings.
  • The PT will then develop and assign a home exercise program to an RA who then sees the client at home multiple times per week to administer the program.
  • PT will then reassess at the mid-way point to evaluate any changes in the client’s mobility status and adjust the home exercise program as needed.
  • A final PT discharge assessment occurs at the end of the 16 weeks to determine if ongoing PT is needed following our program.

Clinical Responsibilities:

  • Responsible for the assessment of clients, establishment and ongoing evaluation and revision of treatment programs, including goals and discharge plans.
  • Maintains current and accurate clinical records according to standards set by the College of Physiotherapists.
  • Documents assessment results, goals, treatment, client progress and discharge plans.

Requirements:

  • Maintain active good standing with the Provincial College of Physiotherapists.
  • Participate in ongoing internal and/or external education activities.
  • Adhere to Bayshore Healthcare / Bayshore Therapy & Rehab Policies and Procedures.

Qualifications:

  • Education: Masters Degree or recognized equivalent in Physiotherapy.
  • Experience: Prior experience with in-home assessments and treatments.

Primary Location: ON-Brockville
